General Instruction to Examinees found in your NOA


Back of the NOA.



Check and do not ignore the back part of your Notice of Admission (NOA). Below are important instructions that should be observed at all times during the examination. A similar instruction will be reiterated once the program of licensure exam for Psychometrician will be out containing the schedule/date/time and particular subjects that will be taken up on 28-29 October 2014.


General Instruction to Examinees 

1. Bring this Notice of Admission (NOA) every examination day.


2. Attend to your personal needs before the start of examination in every subject. No examinee will be allowed  to go out of the examination room while the examination is in progress.

3. Stop answering the test questions at the end of the time allotted for the subject. Arrange your test papers as follows:
            a) Notice of Admission
            b) Answer Sheet
            c) Test Questionnaire

4. Do not leave the room until:
            a)  your test papers are received by the Room Watcher,
            b) you have signed, indicated the time and set (A or B) on the Examinees Record of Attendance 
            c) the lower portion of your Notice of Admission (Certification on the Receipt of Test Papers) is
                signed by the Room Watcher and returned to you.

Prohibited Acts

1. Accepting or receiving anything, including food from any person while the examination is in progress.

2. Giving money, food, or any favor and other consideration to the Room Watchers and other examination personnel.

3.  Loitering, talking, or discussing your answers inside the room or along the corridor while the exam is in progress.

4. Taking out of the examination room any questions used in the examination, copying, and or divulging or making known the nature or content of any examination question or answer to any individual or entity. When you are through with the examination, return unused answer sheets or any other test materials furnished you by the Room Watcher.

5. Copying or referring to any solution, answer or work of another examinee or allowing anyone to copy or refer to your work, helping or asking help from any person or communicating with anyone by means of words, signs, gestures, codes and other similar acts which may enable you to change, impart or acquire relevant information.

6.  Bringing inside the examination rooms the following: books, notes, review materials and other printed materials containing principles or excerpts thereof, coded data/information/formula which are relevant to or connected with the examination subject, programmable calculators, cellular phones, beepers, portable computers or other similar gadgets/devices. The act shall be considered cheating and/or act of dishonesty and shall be a ground for the cancellation of your examination papers and suspension or debarment from future licensure examinations. (PRC Resolution No. 463 dated November 27, 1995).


Maintain discipline at all times. Any misconduct or irregularity on your part or any violation of the examination rules and regulations and instructions 
will be sufficient cause for the cancellation of your examination papers and your debarment from 
taking any future licensure examination.

Petition-Survey for a Regional Psychometrician Licensure Exam



Some of our readers have read in our blog post that group of schools could write to their Professional Regulation Commission - PRC Regional Director to request for a regional licensure exam in their locality addressed to the Board of Psychology and the PRC in Manila. So we are doing this survey to get a sense of how many examinees would be interested to take licensure exam in their region. We encourage in particular those who are based in the Visayas and Mindanao to answer this survey if interested to join this petition/survey. Depending on the number of examinees, the capacity of regional offices to conduct the licensure exam, etc. would be considerations will be taken into account by the Board of Psychology and PRC to decide whether to  approve the conduct of a regional licensure exam. So this petition/survey serves as a possible gauge to endorse to PRC such request. Please note that this is an initiative of TR Reviewer based on the initial feedback we are receiving from our previous blog post on the PAP organized Orientation on the Psychology Licensure Exam held at PSSC last 18 June 2014.

Please share and inform your school school dean and faculty head about this petition for the possibility of a Regional Psychometricians Licensure Exam.

Checklist of Requirements for Psychometrician Licensure Exam

Updated - September 2, 2014

FAQ - Which document should have a documentary stamp? How many documentary stamps are needed? Where to get and how much?

Answer: Documentary stamp should be affixed to your 3 Certificates of Good Moral Character. Original copies will be submitted. But in case your are submitting a certified true copy of your Certificate of Good Moral Character (in the case of a half page school issued certificate - not printed on a bond paper so it can't contain the notary), it will serve as as original copy and can be submitted to PRC - still the original (ex. of the 1/2 page school certificate) should be shown and submitted to PRC together with the other certificates - notarized and with documentary stamp.

You will need 4 documentary stamps - 3 for the certificate and 1 for the application form. Depending on the place where you purchase the documentary stamp, it could be in the office of barangay captain, post office, or PRC, prices varies (it costs 35 pesos from vendors at PRC).

FAQ - Is Barangay Clearance same with the Certificate of Good Moral character?

Answer - Per PRC, for as long as the word "of good moral character" appears in the Barangay Clearance then the clearance can serve as an equivalent of the Certificate of Good Moral Character from the Barangay.

Chin Bona Hi Sir! Just want to share my experience since I went to PRC yesterday. The PRC didn't accept my Good Moral Character from my School since there was no good moral key words stated on the content. The school has a different format and content which states that I was not involved in any disciplinary act but the PRC didn't accept it. I don't know what to do because when I asked the Registrar of my school, they said that they won't change the content of the good moral that has given to me since they have their own format and that it can't be changed.

So always try to follow to the letters those that are being required by PRC otherwise you will keep going back complying their requirements.

Re certificates if you are not yet employed then get fromschool, church and your barangay (one certificate each).



With the PRC email that was sent to all those who already got their NOA, and with calls and email received from PRC that clarified the matter please be guided of this new and updated set of requirements, especially for the benefit of those who are yet to submit their application:

1) On the Certificates of Good Moral Character:

a) 3 Certificates each from school, work, barangay or church, most of the time these certificates contain a dry seal or printed in a special paper or letterhead of issuing institutions. (Re Barangay Clearance instead of Certificate of Good Moral Character - please check it with PRC if they accept Barangay Clearance in lieu of the Certificate of Good  Moral Character.)

b) Certificates should be notarized by a notary public (dry seal does not mean it is notarized already).

c) Each of the notarized good moral certificates should contain each a documentary stamp. You can purchase documentary stamp at PRC (more expensive) other alternative sources - from your city/municipal hall, barangay hall, or the post office.

The email of PRC was mass distributed even tho those whose TOR already contained a SO (special order). But it seems individually they have replied to calls and email verifying on their email that CAV only apply  to those TOR without SO. Check with your school and get the endorsement for CHED application of CAV.  CHED is the only government agency that releases SO. Some schools with their liaison officers are in charge of facilitating the application of the CAV of their students and alumni, for a fee. It take 3 weeks for the CAV to obtain. So for those whose TOR does not contain a SO make sure to apply it from your school or get endorsement for you to apply to CHED. Remember deadline for the application for the licensure Exam is on October 8, 2014.

Although the Professional Regulation Commission (PRC) and the Psychology Board has not released the guidelines and requirements for the application for Psychometrician Licensure Exam, per IRR of the law provides for the following:

SEC.13­A. Documentary Requirements to the Licensure Examination of Psychometricians. All applications shall be filled in the Application Division of the Commission and the qualified examinees shall be issued with notices of admission to take such examination upon submission of the following documents.

(a) Original and photocopy of Certificate of Live Birth in NSO security paper in case of
Filipino citizen; or in case of a foreign citizen, a copy of the law of the state or country which
permits Filipino Psychometrician to practice on the same basis as its subject or citizens, duly
authenticated by the Philippine embassy or consulate therein;

(b) Marriage Contract in NSO security paper for married female applicants;

(c) Original and photocopy of transcript of records (with scanned picture) indicating the
Special Order (S.O.) number; and where school is exempted from the issuance of an SO., a
Certificate of Authentication and Validation (CAV) from the CHED;

(d) Original and photocopy of valid NBI Clearance;

(e) Three (3) certificates of good moral Character, preferably from school, employer,
Church, barangay captain, duly sighed by the issuing authority and duly notarized under oath;

(f) Two (2) colored passport size pictures with white background and complete name tag;
and



(g) Community Tax Certificate.

Just submitted my online application for the Psychometrician Licensure Exam

Below is the screen capture/photo of my online application form for the Psychometrician Licensure Exam set for October 28 and 29. The online application is now live - ONLINE!

Be sure to have your cedula - community tax certificate before applying and filling up the online form.

Next step will be - submitting the requirements at the PRC. But I need to satisfy first the requirements below.
 
From the IMPLEMENTING RULES AND REGULATIONS OF RA 10029

SEC.13. Qualifications of Applicants to the Licensure Examination of
Psychometricians. Any  person may apply to take the examinations as a psychometrician after
furnishing evidence satisfactory to the Board that the applicant:
 
(a) Is a Filipino citizen, or a permanent resident or a citizen ot a foreign state/country which
extends reciprocity to the Philippines relative to the practice of the profession;
(b) Holds al least a bachelors degree in psychology conferred by a university, college or
school in the Philippines or abroad recognized/ accredited by the CHED and has obtained
sufficient credits for the subjects covered in the examination,
(c) Is of good moral character; and
(d) Has not been convicted by final judgment of an offense involving moral turpitude.

Step-by-step Tutorial for the PRC Online Application for Psychometrician Licensure Exam

Well for those among you who are excited like me in taking the licensure exam I might as well share some info here that I hope will be helpful. Particularly beneficial to some of our readers who are in far flung areas away from the Professional Regulation Commission Office and Regional offices. It is not easy to call PRC especially perhaps with the thousands of clients they deal with (hope they install more phones and employ more staff to answer public calls). Well also, it is the challenge of those of us doing self-review, unlike when you join a review center your life would be lot more easier and you be lot more informed (some review centers would facilitate processing their reviewers application), so for those wanting a list of review centers check this link  http://psychometricpinas.blogspot.com/p/pyschology-review-centers.html - I have not availed any of these review centers services, since this blog serves to inform so we are providing this list - which needs to be updated. 

After several calls I was able to talk to a lady at the of the Application for Examination Unit at PRC. She informed me that it might be August when they will have the Psychometrician online application ready. I just hope it is not true, hope they will not make examinees cram applying so close the examination date.

So for those who have access to the internet, here are steps, a sort of tutorial (but for now until close the exam date try to regularly monitor PRC website for announcements) on how to do go about the online application for the Psychometrician Licensure Exam or board exam this coming October 28 and 29, 2014. I learned that there's no use of lining up at PRC if you have not accomplished the online application. 


So apply first online and then visit PRC office near you.

Name of Exam - click down menu scroll - look for Psychometrician (as of writing 28 February 2014 - is not yet available)
- and provide the rest of the information needed
- fill all necessary info needed especially those with asterisk (you will be prompted should you missed any info)

Step 7 - Click Save, then you will get the following information

Documentary Requirements

Original and photocopies of Transcript of Records with Special Order and Date of Graduation, with scanned pictures and with remarks “FOR BOARD EXAMINATION PURPOSES ONLY”. Graduates of government schools and institutions/programs accredited by recognized accredited agencies under the FAAP are exempted from SO. Graduates of New Schools/Degree Programs must submit School Recognition and/or Permit to Operate.

Original and photocopy of NSO-issued Birth Certificate (if NSO copy is not clear, bring copy from the Local Civil Registrar)

For married females, original and photocopies of NSO-issued Marriage Contract (if NSO copy is not clear, bring copy from Local Civil Registrar)

Two (2) passport-size colored pictures with white background and complete name tag

Current Community Tax Certificate (Cedula)

Other specific requirements as required by the Commission or the Professional Regulatory Board.


How to Apply

Before filling out your application form, please take time to read the following instructions and tips:

Accomplish the one-page application form by filling out all of the blank fields in the application form. Fields marked with a red asterisk (*) are required. Submission will not be successful until all required fields are properly accomplished.

TIPS:
Use the scrollbar to select the desired option (ex. name of examination, date, place, etc.)
Click the magnifier  Search icon to search for specific information such as city/municipality, school name and degree/course. When the search result is shown, click on the desired information to select the same. Use simple keywords when searching (ex. to seach for Don Mariano Marcos State University-Bambang Campus, type "mariano" or "bambang").
Dates are entered in a specific format "mm/dd/yyyy" (ex. date of birth "01/30/1980")
For applicants without a middle name, just key in a period (.)
For SPLBE applicants without Cedula, you may use your passport number.

When the encoding is completed, click the Save button, and then OK. A notification message will appear on screen confirming that you have successfully submitted your application. A similar notification will be sent to your email (if you provided your email address).

Proceed to the nearest PRC Office and present all the documentary requirements for assessment, payment of prescribed fees and issuance of Notice of Admission (NOA), BEFORE the deadline for application.

On November 11, 1962, a small group of psychologists gathered at Selecta Restaurant along C.M. Recto Avenue for what would later be considered one of the most defining moments in Philippine Psychology. They came from different institutions, drawn to that single place by a common purpose. From the University of the Philippines (UP) came the Chairperson of the country's first Department of Psychology, Dr. Agustin Alonzo, Fr. Jaime Bulatao, S.J. founder of the Department of Psychology and the Central Guidance Bureau of the Ateneo de Manila University, was also present. With them were Dr. Sinforoso Padilla, who established the Psychological Clinic at UP, and Dr. Jesus Perpinan, who founded the Psychological Clinic at Far Eastern University. Dr. Mariano Obias was also present at that historic meeting presided over by Dr. Alfredo Lagmay of UP, which saw the birth of the Psychological Association of the Philippines (PAP).
Five decades hence, what started as a handful, now number thousands. Today, the PAP is the premier association of psychologists in the Philippines.

PAP is an internationally recognized leader in Psychology in Southeast Asia.
MISSION STATEMENT:

The Professional Regulatory Board of Psychology is a collegial body under the administrative control and supervision of the Professional Regulation Commission (PRC). It is composed of a chairperson, and two (2) members appointed by the President of the Philippines from a list of three (3) nominees for each position recommended by the Commission.

Qualification of Board Members

The Chairperson and Members of the Board shall, at the time of their appointment and for the course of their term, possess the following qualifications:
  1. A natural born citizen and resident of the Philippines;
  2. At least thirty-five (35) years of age;
  3. Possess good moral character;
  4. Hold a doctorate degree in psychology conferred by a university, college or school in the Philippines or abroad duly recognized and/or accredited by the Commission on Higher Education (CHED);
  5. Have at least ten (10) years of practice in psychology and psychometrics in a duly recognized institution, clinic or center, as well as at least five(5) years of teaching experience in the field of psychology;
  6. Be a registered and licensed psychologist, except in the case of the first members of the Board who shall automatically be conferred a valid certificate of registration and a valid professional identification card in psychology and psychometrics upon appointment to the Board;
  7. Is neither an officer, trustee nor member of the faculty of any university, college, institute or school where a regular course in psychology is offered or taught or review classes conducted and shall not have any pecuniary interest, direct or indirect, in any such institution;
  8. Is not an officer, nor hold any position other than being a member of the integrated and accredited national organization of psychologist; and
  9. Shall not have been convicted by final judgment of any crime involving moral turpitude. The members of the Board shall have a term of office of three (3) years or until their successors shall have been duly qualified and appointed. Each member of the Board may be reappointed for one (1) full term of three (3) years. The first members of the Board shall hold office for the following terms: the Chairperson for three (3) years; one (1) member for two (2) years; and the other member for one (1) year, which shall be specified in their respective appointments. Any vacancy occurring within the term of a member shall be filled for the unexpired portion of the term only. Each member of the Board shall qualify by taking the proper oath prior to the performance of his/her duties.

The Board shall have the following powers and duties:
  1. Administer and Implement the provisions of R.A. No. 10029 and promulgate as well as revise or update, as necessary, rules and regulations, resolutions and guidelines hereto: Provided, that the policies, resolutions, rules and regulations issued or promulgated by the Board shall be subject to review and approval of the Commission.
  2. Supervise and monitor the registration, licensure and practice of psychologists and psychometricians in the Philippines;
  3. Administer oaths in connection with the administration of R.A. No. 10029;
  4. Issue, and upon compliance with due process requirements, suspend or revoke, and reinstate, the certificate of registration and professional identification card for psychologists and psychometricians;
  5. Adopt an official seal of the Board;
  6. Monitor the conditions and circumstances affecting the practice of psychology and psychometrics in the Philippines and adopt such measures as may be deemed lawful and proper for the enhancement and maintenance of high professional, ethical and technical standards of the profession;
  7. Issue permits to and exercise visitorial powers over agencies, institutions, associations, corporations and partnerships to verify that the person practicing psychology and psychometrics therein are psychologists and psychometricians with valid certificates of registration and valid professional identification cards, and that they possess the necessary accreditation, skills and/or facilities to competently carry out their functions;
  8. Assist the Commission in the formulation and implementation of the guidelines on continuing professional education for psychologists and psychometricians;
  9. Ensure, in coordination with the CHED, that all educational institutions offering the course/program of psychology strictly comply with the policies, standards and requirements prescribed by the CHED for such course/program, especially in the areas of administration, curriculum, faculty, library and facilities;
  10. Prepare, adopt, issue and amend, in consultation with the CHED, syllabi for the licensure examination subjects;
  11. Investigate and, when warranted, hear administrative cases involving violations of this Act, its implementing rules and regulations as hereinafter promulgated, and any applicable code of ethics and/or code of professional standards, warranting issuance of subpoena testificandum and subpoena ducestecum to secure the appearance of witnesses and the production of document in connection therewith; Provided, That the Board’s decision, resolution or orders rendered in administrative cases shall be subject to review only on appeal; and
  12. Perform such other functions and duties as may be lawfully delegated to it, or as it may deem necessary to carry out the objectives of R.A No. 10029
  13. The Board in the performance of the above-mentioned powers and duties shall seek the assistance of the offices, divisions, sections, and units of the Commission.

President Benigno Aquino III recently appointed Dr. Miriam P. Cue, former Chairperson of the Department of Psychology, as one of the first members of the Professional Regulatory Board of Psychology for a term of two years.

Dr. Cue holds four certificates in Clinical Psychology, Assessment Psychology, Counseling Psychology, and Industrial/Organizational Psychology from the Psychological Association of the Philippines (PAP). Cue has been a long-time consultant in organizational-related problems for non-government organizations, private mental and drug rehab institutions in Mindanao, and industries.

She was actively involved as one of the cluster heads in the psycho-social interventions for Sendong survivors in Iligan City which work was supported by the World Health Organization (WHO).
Dr. Cue officially assumed office on May 22. Consequently, she has opted for an early retirement from active service in the Institute to comply with the provisions of laws that govern her appointment.

Psychometricians and Psychologists Online Application for Registration Without Examination

 


The grandfather clause of  RA 10029 or the Philippine Psychology Act of 2009 provides for exemption of examination as contained in the following sections of the said law:


Section 16. Registration Without Examination for Psychologists. - A person who possesses the qualifications required to take the examination for registration as a psychologist pursuant to the provisions of this Act may be registered without examination: Provided, That the applicant files with the Board within three (3) years after its creation an application for registration and issuance of certificate of registration and professional identification card by submitting credentials satisfactory to the Board that the applicant had, on or prior to the effectivity of this Act, fulfilled the requirements under either subparagraphs (a), (b) or (c) herein:

(a) Obtained a doctoral degree in psychology and had accumulated three (3) years of work experience in the practice of psychology;
(b) Obtained a master's degree in psychology and accumulated a minimum of five (5) years of work experience in the practice of psychology;
(c) Psychologists or employees who hold positions as psychologists presently employed in various government or private agencies, who have a bachelor's degree in psychology, accumulated a minimum of ten (10) years of work experience in the practice of psychology as a psychologist, and who have updated their professional education in various psychology-related functions.

Section 17. Registration Without Examination for Psychometricians. - A person who possesses the qualifications required to take the examination for registration as a psychometrician may be registered without examination:Provided, That the applicant files with the Board within three (3) years after its creation an application for registration and issuance of a certificate of registration and professional identification card by submitting credentials satisfactory to the Board that the applicant before the effectivity of this Act had obtained a bachelor's degree in psychology and had accumulated a minimum of two (2) years full time work experience in the practice of psychometrics.

Online Application for Registration Without Examination

This facility is for the exclusive use of applicants for registration without examination and is available up to:
  • May 21, 2015 - for Psychologists and Psychometricians

Please read carefully the entire instructions and information provided below so you will be guided properly on the procedures, requirements and timetables for registration without examination.
  1. Before submitting an application for registration without examination, the applicant must:

    1. Decide what the application for registration without examination is for (whether for Psychologist or Psychometrician or both). Separate forms and payment will be required for each application.
    2. Determine whether applicant satisfies the qualification standards for Psychologist and Psychometrician. 
    3. Determine if applicant can satisfy all the documentary requirements within 15 days from the time of online application. The documentary requirements for Psychologist, Psychometrician, or Respiratory Therapist are provided hereunder:

      For Psychologist

      1. Original and Photocopy of Certificate of Live Birth in NSO Security Paper for Filipino citizens. In case of a foreign citizen, a copy of the law of the state or country which permits Filipino Psychologists to practice on the same basis as its subject or citizens, duly authenticated by the Philippine embassy or consulate therein.
      2. Original and photocopy of Marriage Certificate in NSO security paper (for married female only)
      3. Original and photocopy of Transcript of Records with scanned picture and with the Special Order Number indicated (for both undergraduate and graduate degrees). Where school is exempted from the issuance of an SO, a certificate of authentication and validation (CAV) must be secured from CHED.
      4. For those who do not have a master's degree, a certified true copy of of at least 100 hours of updating seminars and workshops attended from June 2005 to June 2010 must be presented/submitted
      5. Original Certificates of Practice/Work Experience(s):
        1. Certificate of Employment from immediate superior duly noted by either the HR Manager or Employer specifying the position title, nature of work and specific period of employment duly sworn in by the issuing authorities. Official Job Description signed by the HR Manager must be attached, and for government employees, the official service record specifying the position item must also be submitted.
        2. For those who are self-employed, applicant must submit the following:
          1. Certificate of private practice from colleagues, professional partners and/or institutional clients, specifying the nature of work/services rendered and the duration thereof, duly sworn in and notarized under oath
          2. Work contract(s), if any, and/or sworn in statement of the practitioner specifying the nature, scope and duration of project engagement or services rendered, and the regularity of service-delivery with the undertaking that documentary evidence will be produced when required by the Board
          3. Business permit and DTI registration
      6. Three (3) Original Certificates of Good Moral Character from any of the following: school, employer, church, and/or Barangay Captain duly signed by issuing authority and notarized under oath
      7. Original Certificate of Mental Health or Mental Fitness from a Psychiatrist or from a Registered/Certified Clinical Psychologist, duly signed and notarized under oath
      8. Original and photocopy of valid NBI Clearance plus Ombudsman clearance for government employees
      9. Two (2) colored passport-size picture with white background and complete name tag
      10. Photocopy of Community Tax Certificate

      For Psychometrician

      1. Original and Photocopy of Certificate of Live Birth in NSO security paper for Filipino citizens. In case of a foreign citizen, a copy of the law of the state or country which permits Filipino Psychometricians to practice on the same basis as its subject or citizens, duly authenticated by the Philippine embassy or consulate therein.
      2. Original and photocopy of Marriage Certificate in NSO security paper (for married female only)
      3. Original and photocopy of Transcript of Records for Bachelor's Degree in Psychology (AB/BS) with scanned picture. Special Order Number must be indicated. Where school is exempted from the issuance of an SO, a certificate of authentication and validation (CAV) must be secured from CHED.
      4. Original certificate of full-time work experience as a Psychometrician or full time work engagement in the practice of psychometrics for a minimum of 2 years, either from immediate superior (if employed) or from institutional clients, professional partners/colleagues (if on private practice), specifying the nature of work, duly sworn in and notarized under oath. Official job description must be attached.
      5. Original and photocopy of valid NBI Clearance; plus Clearance from the Ombudsman for those in government service.
      6. Three (3) original Certificates of Good Moral Character, from any of the following: school/former professor, employer, church, and/or Barangay Captain duly signed by issuing authority and notarized under oath
      7. Two (2) colored passport-size pictures with white background and complete name tag
      8. Photocopy of Community Tax Certificate


      NOTE: For applicants working abroad, the required documents must be certified by the Philippine Overseas Labor Office near the place of work.

  2. As soon as applicant has ascertained (1) which registration without examination to apply for (as Psychologist, Psychometrician or both); (2) that he/she satisfies the qualification requirements; and (3) can satisfy all the documentary requirements within the time stated, the applicant is ready to proceed to submit his/her application online.

  3. After receiving an acknowledgement through applicant's email that the application for registration without examination has been submitted successfully together with an order form for payment, applicant must follow the steps hereunder:

    1. On the assigned date, applicant must go to the Customer Service Center of the nearest PRC office and secure one (1) P15 metered documentary stamp. Affix the documentary stamp on the space provided in the application form, sign it and indicate the date when the application form was accomplished. Applicant can then proceed to the cashier's window for the payment of the application/processing fee (P900.00 for each application to be processed).

    2. Submit a copy of the official receipt (OR) evidencing payment together with the duly accomplished application form and documents to:

      • For Psychologist and Psychometrician - Window 12, Application Division at the Ground Floor of PRC Main Building
      • PRC Regional Offices

      Submit all required documents in a clear book with the confirmation number and official notice of submission from the PRB on the first page, a table of contents on the second, the printed application form on the third page, and the rest of the documents in subsequent pages arranged according to the list provided.

      The application processor will conduct an initial screening of the application documents and should inform the applicant of any deficiency at this time. Applicant should receive a claim stub after initial screening is done.

    3. After receipt by PRC of applicant's documents, he/she should wait for official notification from PRC through his/her email account. Applicant is also advised to check the PRC website regularly to check whether the application has been approved or call tel. no. 3101018 to inquire about the status of application.
